You will need to have the Adobe acrobat reader To view or print the preliminary application form in PDF format  If you don't have adobe reader installed, you can download it for free from the adobe website. 

Complete the preliminary application and fax it to 347-438-2961 . Please make sure that you complete all parts, sign and date the form before you fax it.

1. Use your right mouse button to click the the link app.pdf and choose the save target as option.
2. Point to a location that you want the form to be downloaded to your computer. (choose desktop for easy access)
3. Double click the form on the desktop to open/ view and print ( you must have adobe to view/print)
